Job Description

Responsible for the overall coordination of the referral and admissions process. Interacts closely with facility representatives, discharge planners, and other agencies to facilitate the initiation of care.

Skills / Requirements

Position Qualifications:

  • Be eighteen (18) years of age or older
  • Minimum of 1 year experience in a healthcare related field; hospice and/or homecare experience preferred.
  • Ability to manage multiple concurrent tasks and prioritize assignments
  • Excellent in both written and oral communication skills, including spelling and grammar.
  • Ability to handle stressful and delicate situations with tact, diplomacy, and attention to their importance.
  • Ability to assist with completing admission paperwork.
  • Must represent Ambercare in a positive and professional manner.
  • Must have a valid NM Drivers license.
  • Must have reliable transportation available during work hours.  Vehicle must be properly registered and insured in accordance with state law.

Essential Job Functions/Responsibilities:

  • Receive referrals by phone or in person for designated department.
  • Provide explanation of the benefits to patients, families, and community contacts.
  • Referral sources with finding alternative services when patient is not a candidate for Ambercare.
  • Oversee scheduling of initial nursing assessment and admission.
  • Maintain admission packets and ensure that proper forms are available to admissions staff.
  • Knowledgeable in all Home Health and/or Hospice processes.
  • Community outreach representatives with marketing activities to promote relations with referral sources.
  • Completion of admission paperwork as needed.
  • Provide telephone and clerical support to other staff members as time allows.
  • Adhere to dress code.
  • Adhere to work schedule and maintain good punctuality and attendance record
  • Other duties or specific assignments as directed by department supervisor.
