Job Description

The hospice aide is a paraprofessional member of the interdisciplinary team who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patients personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting the observations, and documenting observations and care performed.

Skills / Requirements

 QUALIFICATIONS:

1. Be eighteen (18) years of age or older
2. Successful completion of a nurse aide training program and competency evaluation.
3. Successful completion of Ambercare home health aide competency exam and skills demonstration
4. Must be able to perform physical demands of patient care including lifting and transferring of patients
5. Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
6. Is self-directing with the ability to work with little supervision. Secure with issues of death/dying. Provides a calm manner when in a patients home 
7. Must have a valid NM Drivers license
8. Must have reliable transportation available during work hours.  Vehicle must be properly registered and insured in accordance with state law.
9. Must possess current CPR Certification

DUTIES AND RESPONSIBILITIES:

1. Provide essential personal care assistance to patients consistent with policies and procedures of Ambercare.
2. Document visits mileage etc. electronically with Celltrak
3. Providing patient and family with positive communication techniques, within realm of ethical a respectful care, including confidentiality.
4. Document care given in accordance with policies and procedures of Ambercare
5. Assessing and reporting on patients condition and significant changes to the Case Manager. Also aware of caregiver or other individuals living with the patient and interpersonal issues.
6. Attend at least 12 hours of in-service training per year
7. Attend and participate in interdisciplinary team meeting as requested
8. Provide emotional support and compassionate care to patients and families

9. Provide personal care including baths, back runs, oral hygiene, shampoos and changing bed linen as often as assigned.
10. Keeping patients living area clean and orderly, as assigned

12. Assist in feeding the patient as assigned
13. Practice universal precautions when caring for patients
14. Provide incidental household services as are essential to the patients well being at home.
15. Take and record vital signs as directed by the Nurse Case Manager
16. Adhere to dress code
17. Adhere to work schedule and maintain good punctuality and attendance record
